Census and 5500 Data Request Downloads To complete year-end testing, contribution allocations and Form 5500, ATR requires certain information. Click on the "Download Now" link next to the document you want to download. You will need the Adobe Acrobat Reader on your computer to open the file types PDF. If you do not have the Adobe Acrobat Reader, you may download it for free from www.adobe.com. You will need Microsoft Excel Version 4.0 or higher to use the Census Data Collection Template. The data requests are shown for the Plan Year ending December 31, 2011. NOTE: If your plan ends on a later date, adjust the dates to match your Plan Year. The Department of Labor (DOL) Reporting and Disclosure Guide for Employee Benefit Plans is also available for download. This Guide provides information about your responsibilities to report Plan information to the IRS, the DOL and to Plan participants. The Guide also provides the deadlines for providing this information. |
